Description

Ciprofloxacin: an Antibiotic and Antimicrobial Agents for Ophthalmic and Urinary Tract Infections

Abstract

The clinical use of ciprofloxacin for ophthalmic and urinary tract infections (UTIs) in children has been studied by researchers. The drug was administered in an outpatient setting, at doses of 250 and 500 mg twice daily for a period of 3 days. The results showed that ciprofloxacin was effective against UTIs at the lowest dose (250 mg twice daily), when administered at the time of diagnosis. The use of the antibiotic in UTIs is a major cause of bacterial resistance, but there is no evidence that the antibiotic is useful for treating UTIs. To our knowledge, this is the first study to show that ciprofloxacin is effective in the treatment of UTIs.

Important Information

Introduction

The first oral antibiotics that were introduced into the market in the 1950s have been effective against bacterial pathogens in the urinary tract, the kidneys, and the eye. A variety of antimicrobial agents have been developed in the form of oral and intravenous formulations, and a large number of studies were carried out, although the effectiveness of these drugs was generally unsatisfactory and their adverse effects were very small. In the course of the last 20 years, there have been many reports about the use of ciprofloxacin in the treatment of UTIs, and studies have been carried out on the use of ciprofloxacin in other patients who had a history of urinary tract infections. In this study, we aimed to investigate the efficacy of ciprofloxacin in the treatment of UTIs in children.

Materials and Methods

A systematic review of studies was carried out to identify all clinical trials involving ciprofloxacin in the treatment of bacterial infections in children. A total of 11 studies with 5,851 children were included in the meta-analysis. These studies were conducted in the pediatric department of our institution, according to the definition used by the study. Inclusion criteria were as follows: 1. Diagnosis of an UTI; 2. Acute otitis externa; 3. Acute uncomplicated UTI; 4. Acute bacterial sinusitis; 5. Acute uncomplicated urethritis; 6. Acute bacterial prostatitis.

Results

In the meta-analysis, the most commonly prescribed antibiotic among the top five medications prescribed in children for the treatment of acute UTIs was ciprofloxacin, followed by levofloxacin and moxifloxacin. The risk of bias for each antibiotic was low in all studies.

Conclusions

In the present study, ciprofloxacin showed a significantly higher efficacy than the other antibiotics in the treatment of acute UTIs.

Introduction

Ciprofloxacin hydrochloride, a fluoroquinolone antibiotic, is used to treat bacterial infections, including urinary tract infections (UTIs), respiratory tract infections, gastrointestinal infections, and infections of the skin and soft tissue (e.g., cellulitis and exfoliative dermatitis). Its use is also beneficial in treating bacterial infections of the ear, nose, throat, urinary tract, and gastrointestinal tract (e.g., infections of the lower respiratory tract, urinary tract, and reproductive tract).

In some instances, ciprofloxacin hydrochloride is also prescribed for purposes other than those outlined in this section.

Mechanism of Action

The ciprofloxacin hydrochloride antibiotic binds to the bacterial DNA polymerase enzyme, DNA gyrase and topoisomerase IV (DNA gyrase and topoisomerase IV) and inhibits DNA replication. The binding of the drug to these enzymes results in the synthesis of a double-stranded linear DNA chain that can be incorporated into bacteria cells by a bacterial DNA polymerase. This process results in the death of the bacteria and, ultimately, the elimination of the infection.

Mechanism of Action of ciprofloxacin hydrochloride

Infection and Cellulitis

Ciprofloxacin hydrochloride, the active ingredient in ciprofloxacin hydrochloride, is bacteriostatic. It inhibits the bacterial cell wall synthesis by inhibiting DNA gyrase and topoisomerase IV. This inhibition results in the death of the bacterial cells. Ciprofloxacin hydrochloride is effective against bacteria that are susceptible to antibiotics.

Ciprofloxacin hydrochloride is bactericidal in nature and causes a rapid and severe decrease in the growth of bacteria. The growth of these bacteria is usually not observed for weeks or months following treatment. In addition, the growth of other bacterial strains and the production of other substances that cause bacterial infections may also occur.

Ciprofloxacin hydrochloride is not effective against Gram-positive and Gram-negative bacteria that are known to be susceptible to ciprofloxacin hydrochloride. Ciprofloxacin hydrochloride is bactericidal and causes a rapid decrease in the growth of susceptible bacteria. This decrease in bacterial growth is usually observed for days or weeks after the first administration of ciprofloxacin hydrochloride.

Pharmacokinetics

Ciprofloxacin hydrochloride is excreted primarily in the urine (0.5-1 mg/mL) by the gastrointestinal tract. It is metabolized by the liver, and this elimination process is not affected by the presence of food in the form of fat or food products. Ciprofloxacin hydrochloride is bound to albumin, and it is less stable in the blood.

It is known that ciprofloxacin hydrochloride is bioavailable in the form of tablets, capsules, and suspensions. The active ingredient in these tablets and capsules is ciprofloxacin hydrochloride.

The following table compares the bioavailability of ciprofloxacin hydrochloride versus that of tablets and capsules. Tablets are taken orally as tablets and capsules with a loading dose of ciprofloxacin hydrochloride.
